Terms and Conditions
Price and Delivery.
Unless otherwise stated, prices do not include taxes, duties, insurance or freight charges. Quoted prices are effective for 30 days from date of quote. Shipping dates will be established at the time buyer’s order is accepted. All purchase orders are to be sent via email to: customerservice@slaterendoscopy.com
Payment and Risk of Loss.
Unless payment is by credit card, payment in U.S. dollars is due within 30 days of shipping invoice. Any invoice over 30 days will be charged an additional 2 ½% of the total invoice price. All shipments will be FOB Destination, and all risk of loss or damage will pass to buyer upon Seller’s (Slater Endoscopy) delivery of goods to the Buyer’s destination. Freight is paid by buyer and each purchase order shall include the buyer’s UPS or FedEx shipper number.
Disclaimer of Warranty and Limitation of Remedy.
There is no express or implied warranty, including any implied warranty of merchantability or fitness for a particular purpose, on the Slater Endoscopy product(s). Under no circumstances shall Slater Endoscopy be liable for any direct, incidental, or consequential damages other than as expressly provided by specific law. No person has the authority to bind Slater Endoscopy to any representation or warranty accept as specifically set forth herein. Descriptions or specifications in Slater Endoscopy printed matter, including this document, are meant solely to generally describe the product at the time of manufacture and do not constitute any express warranties.
Product Return Policy
To be eligible for any return, the buyer must submit a completed (Product Return Request Form) no later than 30 days after the delivery date of the product that is returned. The return limitation period that applies to the return depends upon the condition of the products that are returned (new/unused versus damaged), as further explained below.
All product return requests are initiated by completing and submitting a Product Return Request Form. The buyer may request the Product Return Request Form from Slater Endoscopy, and complete the form and submit via email to: customerservice@slaterendoscopy.com
When completing the Product Return Request Form, the buyer must provide all required information accurately and correctly where indicated. A failure to do so may cause delay or rejection of the request. The buyer is solely responsible for the accuracy and correctness of any information that is to be provided. (Contact Slater Endoscopy if there are any questions related to completing the form).
All product returns must have a Slater Endoscopy assigned Return Authorization Number or RA number. After the Product Return Request Form is received and the return is authorized, seller will provide the buyer with a unique RA number that is assigned to the product return.
Product returns may be voided on product shipped or taken outside the United States and are non-transferable in all events.
Seller is not responsible for re-shipments if a buyer or distributor have provided an incorrect shipping address or if entered a P.O. Box number.
Product return rights are non-transferable. This means that to be eligible for any return, the buyer must have been the registered purchaser/user at the time of original purchase. This also means that only the original buyer is entitled to receive replacement product. We will not drop ship to another location or person at the direction of a distributor or a buyer.
Final approval on any product return request is entirely conditioned and contingent upon the inspection of the returned products upon arrival and will depend on whether those products in fact fit the product return criteria.
Return of Unused Product
These additional requirements apply to requests to return unused product.
The buyer must initiate any return of unused product no later than 30 days after receipt of original delivery. All product return requests are initiated by completing and submitting the request on Product Return Request Form.
All returned product(s) must be received by seller in an unused, fit to be offered-for-sale condition in the original individual packaging. Safety seals or sterile packaging cannot be opened or broken; and labels, markings, or packaging cannot be altered.
The buyer must return all parts and accessories in the original packaging.
Seller will not refund or credit the original handling and shipping charges for unused product.
Unused product returns are subject to a 20% restocking fee which will be deducted from the amount that is ultimately refunded to the customer.
Return of Damaged Product
These additional requirements apply to requests to return damaged product.
A damaged product is generally one that arrives on delivery to a buyer in damaged condition and appears damaged or functionality is compromised during the initial inspection.
The initiation of the return of damaged products must be no later than 30 days after the original delivery. All product return requests for damaged products are initiated by completing and submitting the Product Return Request Form.
The buyer must initiate any return of damaged products no later than 30 days after the customer receives original delivery. All product return requests for damaged products are initiated by completing and submitting the request on Product Return Request Form.
The sole recourse in the case of returning a damaged product shall be to receive a replacement product free of charge (including shipping charges) after our receipt and inspection of the returned product.
